Appointment Information

First Visit
Each new patient is asked to complete a health history form before their first visit to NorthEast Pediatric Therapy. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards, including Medicaid cards. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival.

Routine Visit
The goal of NorthEast Pediatric Therapy is to provide patients with prompt, easy access to quality healthcare. Our therapists are available to see patients by appointment during normal office hours. Please arrive on time for your scheduled appointment.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
